Prelim Entry Form and Information

Age Divisions:
Boys: 0-2yrs, 3-5 yrs, 6 & up yrs.
Girls: 0-23 mos, 2-3 yrs, 4-5 yrs, 6-7 yrs, 8-9 yrs, 10-12 yrs, 13-15 yrs, 16-19 yrs, 20 & up (married or single).

*All contestants 0-4 may be escorted on stage by parent or adult.
*Age group is determined by contestants age day of pageant.
*All contestants will be recognized & receive a beautiful tiara and a nice gift!

Beauty (mandatory) entry fee $60.00
We will crown a Queen/King and 3 alternates in each of the 12 age divisions. Our Queen/King will receive a Crown, Banner, Gift and their Beauty Fee PAID to the State Pageant and $25.00 Cash to be awarded when they enter the state pageant. Each alternate will receive a tiara/scepter, gift, and their Beauty Fee PAID to the State Pageant.

Photogenic
Entry fee $10.00
We will award a winner in each of the 12 age divisions. She/he will receive an award gift, and their Beauty fee PAID to the State Pageant. Photos will be turned in at registration. They may be black and white or color. You may enter extra photos at $5.00 each. Photos should be marked clearly with your child's age and name on the back of photo. Photos should not be larger than 8 1/2 x 11 and no smaller than wallet size.

Best Hair, Best Eyes, Best Smile, and Best Personality
Entry fee $20.00 (includes all four)
We will award a winner in each of the 12 age divisions. She/he will receive an award and their Beauty fee PAID to the State Pageant! We will award a Best Hair, Best Eyes, Best Smile, and Best Personality in each age division.

Highpoint Option!! 2 Lucky Winners (0-5 & 6-up) will be awarded $50.00 Cash on stage!! A beautiful crown, banner, gift, plus 1/2 off the Super Saver package at State Finals! Be sure to enter the Super Saver Package to qualify to win CASH on stage!!!
*must be entered in Supersaver.
*must have 15 contestants to award.

Attire: Girls may wear Sunday best, pageant dresses or prom gowns. Boys may wear Sunday best or tuxedo.

Referrals
Receive $5.00 credit for each person that you refer to the pageant. Your name must appear on the referrals form. Refer 5 contestants and enter pageant free (excluding door admission).

Sponsor Tickets
If you would like to sell sponsor tickets to help pay your entry fees, you may do so. Tickets sell for $1.00 each. Please call our office for tickets. Ticket sales must be mailed 5 days prior to the pageant.

How to Enter
Simply mail your entry form and entry fees to the address below or you can enter at the pageant. Please have your entry form filled out. If you are entering at the pageant, all fees will be due CASH ONLY. Please call and let us know if you are planning on entering at the pageant. Everyone will register at this time and turn in their Photogenic entries. If you have any questions please give me a call. I'll be happy to help in any way possible.

This year at Nationals we will be awarding:

3 - $1500.00 Bond 15 - $1000.00 Bond
15 - $400.00 Bond
